Amanda Sorrill, PT

Patient: 92 year old female pt who is 67 in and 168 lbs

Primary Diagnosis: s/p acute colectomy with functional decline, anemia, mild
cognitive impairments, HTN, type 2 DM, chronic kidney disease, PVD, macular
degeneration.

Existing Wheelchair Seating: Pt admitted from the hospital with a significant
functional decline and required the support of a broda chair without a cushion.

Positioning / Clinical Issues: In the broda chair the pt was unable to be effectively
positioned for meals to promote independence with self feeding and safe
swallowing. The broda chair limited the pts self mobilization. The pt sits for
prolonged periods of time in her chair and requires a supportive cushion to enable
her to transition into a sling seat wheelchair.

PostureWorks Intervention: Sling seat wheelchair with a Silver Care Basic.

Outcome: Pt is now in a position that allows for improved independence with self
feeding and safe swallowing. She is able to self mobilize in her wheelchair inside the
facility. She is comfortable and happy.

Quality Outcome Measures Impacted by Intervention: Quality of life, self mobility
and ADLs

Feedback from patient: “I am happy about progressing to a wheelchair, and my
cushion has helped make this transition possible.”

Matrix Medium Profile with Position Pro Insert
Improving Positioning, Mobility, and ADLs

125+ Bed Skilled Nursing Facility

Patient: 87 yo female

Primary Diagnosis: Generalized OA, left femur fx s/p ORIF

Existing Wheelchair Seating: Standard sling seat WC with foam cushion and
high back attachment

Positioning / clinical Issues: Pt presented with increased sacral sitting with
5/10 LBP in prior seating system. The pt was unable to self mobilize her seating
system without assistance, which decreased her freedom of mobility in her unit
and throughout the facility thus decreasing her QOL.

PostureWorks Intervention: Matrix Medium Profile with Position Pro Insert

Outcome: The Position Pro Insert inside of the Matrix Medium Profile cushion
took the hammocking out of the sling seat wheelchair and created a pelvic well.
This placed the pt into pelvic neutral and prevented hip internal rotation and
adduction. As a result, the pt was no longer in sacral sitting and sliding and
demonstrated an upright and midline trunk position. OT was able to remove the
high back attachment and replace with a standard back, and in accordance with
lowering the WC, the pt was able to independently self mobilize WC on unit and
throughout facility to participate in mealtimes and leisure activities with her peers.
The improvements in biomechanical alignment of the pelvis decreased low back
pain from 5/10 to 0/10.

Feedback from GRS therapist (MS, OTR/L): “ Pt is relieved that she no longer
has back pain in her new seating system. Her QOL has also greatly improved
now that she can independently self propel her WC to and from meals and

ENGAGE APODTM CASE STUDY
Improving Positioning & Reducing Pain
Post Acute Care Facility, Maryland

Patient: 53 year old, female, long term care patient

Primary Diagnosis: hemiplegia and hemiparesis, cerebral infarct due to
occlusion or stenosis of cerebral artery, symbolic dysfunction, hyperlipidemia, DM
type 2, abnormal posture

Existing Wheelchair Seating: standard sling seat wheelchair with foam general
use cushion

Positioning / clinical Issues: Pt reported discomfort when sitting in wheelchair.
Immediate right lateral leaning when up in wheelchair limited her ability to self
mobilize in her wheelchair and perform functional tasks. Sitting tolerance was
limited to 2-3 hours on foam cushion due to discomfort. Limited ability to attend
facility activities and leisure activities.

PostureWorks Intervention: Engage APODTM Low Profile cushion

Outcome: Pt now reports no discomfort when up in her wheelchair on the
Engage APOD cushion. She is now sitting in an upright midline trunk position
with no lateral leaning. She is able to independently self mobilize her wheelchair
throughout the facility. She is able to attend leisure activities. She is able to sit up
in her wheelchair for 8-10 hours.

Quality Outcome Measures Impacted by Intervention: Pain, QOL, Mobility

Feedback from patient: When the patient received her cushion, she cried and
thanked the therapy team for providing her a cushion that eliminated her
discomfort and improved her sitting tolerance and quality of life.

Feedback from Dir. of Rehab, PTA: “I am amazed to see the immediate
change in positioning. My patient is in midline and is happy again. Seeing my
patient with tears of joy was amazing.”

WOUND & POSITIONING CASE STUDY
MATRIX and POSITION PRO Insert ®
Brian Center of Southpoint, Durham NC

Patient: 50 year old male admitted to rehab in this SNF following B AKA; patient previously
lived at home.

Primary Diagnosis: Cerebral Palsy, history of chronic Stage 1 and Stage 2 wounds on
sacrum. Patient has been wheelchair bound since 10 years of age.

Existing Wheelchair Seating: Custom manual wheelchair with foam pressure relief cushion
with sacral cut out.

Positioning / clinical Issues: Following acute bilateral AKA, pt demonstrated difficulties with
transfers from foam cushion with sacral cut out. Pt also demonstrated difficulty with self care
for hygiene and dressing as he was unable to effectively weight shift on the foam cushion with
the cut out to perform ADLs.
Pt has extensor tone with weak abdominals resulting in fixed positional abnormalities. Pt is in
a posterior pelvic tilt, increased thoracic kyphosis, with bilaterally rounded shoulders, forward
head posture, and sacral sliding.
Acute Stage 2 wound 2cm by 1cm

PostureWorks Intervention: Matrix medium profile cushion and Position Pro Insert

Outcome: Use of Matrix Medium profile cushion with Position Pro Insert resulted in increased
independence with self care in the wheelchair and improved quality and independence with
transfers in and out of the chair.
The pt is now able to maintain upright posture without sliding anteriorly in the chair.
The patient’s sacral wound closed after using the Matrix Medium Profile and Position Pro Insert
for 3 weeks.

and redistribution, functional mobility with transfers and wound healing

believe this was especially helpful for my patient with bilateral above knee amputations as he

Existing Wheelchair Seating: Patient admitted into facility with a high back
reclining wheelchair and an air flotation cushion.

 

Positioning / clinical Issues: Patient presented with decreased upright sitting
posture secondary to B hip thrusting that decreased patient’s overall level of
independence with self­feeding and interaction within his environment.

 

PostureWorks Intervention: Engage APOD Medium Profile

 

Outcome: Patient now demonstrates good upright sitting posture that is enabling
patient to interact more independently within his environment, and is helping with
his ability to self­-feed.

lateral trunk lean to right

Outcome: –Increased sitting tolerance: little to no lateral lean, little to no trunk rotation,
decreased windswept position
-Increased ADLs: transitioned from assisted to independent dining – pt now feeding
herself
-Increased quality of life: pt now interested in activities and outings; decreased
request to be in bed between meals
