Patient: Male, 89 years old
Primary Diagnosis: Dementia, R hip fracture s/p THR which dislocated and could not be surgically re-located due to medical complexities
Existing Wheelchair Seating: Pt was in a standard sling seat wheelchair with foam cushion
Positioning / clinical Issues: Pt has hip IR as a result of pmhx. Pt was in sacral sitting and sliding which limited his wheelchair mobility and safety. He was at increased risk for skin breakdown due to constant sacral sitting and sliding. Nursing constantly repositioning the pt.
PostureWorks Intervention: Engage Medium Profile Cushion
Outcome: Pt is no longer in sacral sitting and sliding and there has been a significant decrease in number of times per day that nursing needs to reposition the pt. Pt has reduced risk for skin breakdown. Pt demonstrates less R hip IR while on the Engage cushion and self mobilizes easily and independently throughout the facility.
Quality Outcome Measures Impacted by Intervention: Skin, Mobility
